MATERNIX™ Release of Liability, Waiver of all Claims and Assumption of Risk, Consent for Participation

I hereby acknowledge that I have agreed to meet with Tara Campbell, a Registered Physiotherapist who is in good standing with the Ontario College of Physiotherapy and a Health Care Practitioner (HCP), to participate in the MATERNIX™ program for Educational and Informational purposes only. The education instruction and information provided in Tara’s MATERNIX™ program is in no way meant as Physical Therapy.

The online information, emails and products of MATERNIX™ are a resource guide, and are intended for people looking for more information, not a substitute for working with a licensed medical professional. By viewing and participating in this program and /or website I acknowledge that Tara cannot guarantee any particular results, nor that Tara is providing medical advice. I agree to follow any information or recommendations on this website, emails or affiliate sites and participate in the MATERNIX™ program whatsoever at my own risk, and I agree that I will not hold Tara or any individual involved in delivering MATERNIX™ responsible in the event of my incurring an injury and/or exacerbating any previously existing conditions. If I have any prior/current medical conditions, I agree that I have consulted with my physician to ensure it is appropriate for me to participate in the type of education and informational physical activity provided by MATERNIX ™. Further, if I need medical advice or care before, during or after the MATERNIX™ program, I agree to seek it from a qualified medical professional.

I have read and I understand this Release, Waiver and Consent Form. I hereby release Tara Campbell and anyone involved in delivery of the MATERNIX™ program from any responsibility or liability whatsoever due to my participation in MATERNIX ™.

By agreeing to the Terms and Conditions, I am indicating that I have reviewed the foregoing MATERNIX™ Release, Waiver and Consent Form, and I acknowledge and expressly consent to the terms described therein.
